Reusable forms

How it works

From contact details to Mailchimp in three steps.

01

Collect

Give someone a simple form to enter the contact information you need.

02

Complete

Keep the information structured and make it easier to avoid missing data.

03

Download

Download the completed CSV and import the contacts into your Mailchimp audience.

Why use a form?

Getting the contact data is often harder than importing it.

Sending someone a spreadsheet template sounds simple until columns get renamed, email addresses are entered incorrectly, or everyone formats their data differently.

A structured form gives people a clearer way to provide the information you need before importing it into Mailchimp.

Missing contact information

Required information is easier to communicate before the data reaches your audience.

Inconsistent data

Give people structured fields instead of asking them to work inside a spreadsheet.

Spreadsheet back-and-forth

Collect contact information directly through a simple form.

Mailchimp CSV template

A simple way to collect Mailchimp contact information

Use this Mailchimp contacts CSV template to collect information such as email address, first name, last name, phone number, company, job title, website, address, birthday and tags.

Instead of sending someone a spreadsheet and asking them to fill it in, give them a simple form and download the resulting CSV when they're finished.

Mailchimp requires an email address when importing email contacts. Additional fields such as first name, last name, phone number, website, birthday and other audience information can be included in the import file. View Mailchimp's official CSV formatting guidelines.

Mailchimp allows you to upload a CSV file and then match the columns in your file to fields in your audience. This means you can collect the data first and handle the final audience mapping during the Mailchimp import process. Learn more about importing contacts into Mailchimp.

If you're collecting subscribers for email marketing, make sure you have the appropriate permission to send marketing emails to the contacts you import. Mailchimp specifically notes that imported subscribed contacts need appropriate permission. See Mailchimp's import guidance.

If you need different fields or more than three rows, you can create your own Import Builder form and configure it for your contact collection workflow.

Need a contact CSV form of your own?

Create a custom Import Builder form with your own fields, validation and row limits.
