How it works
From a client's fleet list to a submission in three steps.
01

Send the form to the client, or import last year's schedule for them to update.
02
Complete
One vehicle per row. Bad VINs, repeated vehicles and missing ZIP codes are highlighted.
03
Download and submit
Download the CSV and add it to the submission, or paste it into the carrier's schedule.
Why use a form?
A mistyped VIN costs a round trip.
Fleet lists arrive with 16-character VINs, an O where a zero should be, the same truck listed twice and garaging ZIP codes missing their leading zero. A carrier can't decode the vehicle or rate the territory, so the schedule comes back with questions.
A form checks every VIN's length and flags repeats while the client is still entering them.
VINs the wrong length
Every VIN from 1981 on has exactly 17 characters: anything else is flagged.
O for 0, I for 1
VINs never use I, O or Q, and the help text says so where the client types it.
The same vehicle twice
Each VIN can appear once, so a truck copied from two lists is caught.
Commercial auto vehicle schedule
What goes in a vehicle schedule
A commercial auto vehicle schedule lists every vehicle a business wants insured: one row per vehicle with its year, make, model and VIN. Carriers decode the VIN to confirm the vehicle and its weight class, and rate it on where it's garaged, how far it travels and what it's used for.
A VIN has exactly 17 characters for every vehicle from the 1981 model year on, as federal rules (49 CFR 565) define it. It uses the numbers 0 to 9 and every letter except I, O and Q, which would be confused with 1 and 0, and its ninth character is a check digit. Copy it from the title, registration or the plate on the dashboard rather than typing it from memory.
Trucks are usually rated by weight, radius and use. Gross vehicle weight rating (GVW) is on the label in the driver's door frame. Radius is how far the vehicle usually travels from where it's garaged: local up to 50 miles, intermediate 51 to 200 and long distance beyond that. Use is service (tools and crew to job sites), retail (deliveries to homes) or commercial (hauling goods).
List the garaging ZIP code for each vehicle, not the head office's, since territory is rated by where it sleeps. Keep the schedule current through the year: a vehicle added or sold changes the premium, and carriers will ask for the same columns at renewal.
